Women's Health Research Registry™

It ’s Easy to Get Started


Becoming a member of the Women’s Health Research Registry is easy and requires very little time. After you complete a brief questionnaire about your medical and lifestyle history, the information is transferred to our confidential database where all of your information is kept separate to ensure your privacy.

If your information matches the requirements of a new or ongoing research study, the Research Registry coordinator will contact you by mail/email. The information packet will describe the study in detail so that you can decide whether or not you would like to participate.

The first part of the registration process is to electronically 'sign' the informed consent form . This form explains in detail how the registry works. Please review this carefully. If you would like to print out a copy for your records, please click on the "consent form" button in the left-hand section of this screen (under "Programs and Services").

The second step is to create your profile . This section contains your contact information; required fields are marked with an asterisk.

The third and final step is to complete the health questionnaire . This section is important as it provides us the information necessary to "match" you with current studies. There are 19 sections to the questionnaire and should take approximately 15-20 minutes to complete. Please note that you are not an active member of the Registry until all sections of the questionnaire are complete.
